SPORTS PHYSICALS

Sports physicals can be completed online via Healthy Roster OR completed on paper!  See attachments below for specific instructions.  Parents/guardians must fill out sections 1-5 and LASD parental consent form.  Section 6 is to be filled out by a doctor or physician (only MD, DO, PAC, CRNP, or SNP).  PIAA paperwork can be accepted for the upcoming school year after May 1st.

The full PIAA packet must be completed for the first LASD sport your student athlete participates in that school year.  All required forms MUST be completed and handed in to the school, coach, or Alivia by the first mandatory practice date.  

If your student athlete has participated in a school sport earlier in the current school year, a Recertification Form is required.  Your student athlete does not need an updated physical after their initial physical.
