CURRENT STUDENTS must have a signed Transcript Release Form to request a transcript.

Once filled out and signed by a parent, please return it to Mrs. Melanie Meakin in the counseling office, or you can also email it to: meakinme@lasd.k12.pa.us

Copies of this form are also available in the Counseling Office at LHS. 

FORMER STUDENTS can request a transcript one of two ways:

2. Complete the Transcript Release Form
