Volunteer Information
Dear Potential Volunteer,
The attached packet provides to you all of the paperwork which is required in order that your name can be presented to the Littlestown Area School District Board of Education for approval to volunteer in the area which you indicate interest.
Contents of the packet are as follows:
1. School Volunteer Information Sheet- Please complete and return this along with the paperwork
listed below.
2. Pennsylvania State Police Request for Criminal Record Check and Pennsylvania Child Abuse
History Clearance- Complete the entire Section I of each clearance, including your personal
information. Return the Criminal Record Check to the Administration Office and send the
Child Abuse History Clearance to the appropriate correspondence. The District will pay for and
submit the Criminal Record Check. The results will be mailed directly to you. It is very important
that you remember to complete the process by bringing those results into the Administration
Office to be included in your file.
3. School Personnel Health Record - Results of a Tuberculosis Test which was administered less than
one year are required of all volunteers. Please complete Part I, filling in your personal information. Part
III must be completed by your physician upon the administration of a TB test. Only sections I and III
are required. You do not need a full physical.
The District is willing to provide to you a TB test free-of-charge. If you would like to take advantage of
this offer, please call the Alloway Creek Intermediate School Nurse, Paula Frey, at 359-4146, extension
4424, to arrange a mutually-agreeable time.
All four RED items listed above must be completed and turned in to the District Office before your name is
presented to the school board for approval to volunteer.
Please remember that it is very important for you to bring the results of both clearance requests into
the Administration Office as soon as they are received so that these can be included in your file.
